Below inflation salary adjustments. You basically get poorer each year even though you did your best. No career development path in the company. No project ownership. Distributed teams that blame each other on constant basis. Poor leadership. Ambiguity in decision making. Many man-hours are wasted due to bad planning and/or lack of direction. Toxic work culture that promotes people who abuses ladder of leadership.
