Poorly managed, goals are always moving, processes contradict one another, uncomfortable rules of engagement. Nobody knows the processes to deliver service properly when something goes wrong, and rules of engagement make it such that I cannot speak with other team members. I have to relay messages through other people. It's a horrible game of Telephone, the game we played when we were kids. Sales makes promises to clients that implementation cannot keep. Nobody knows the history of the company, or has a clear direction as to how to properly deliver service without failure.

The entire IT group exists to satisfy the ego of the CIO. Business processes in no way represent best practices. The few people in leadership who understand this won't change anything. Nobody rocks the boat. We continue to do some of the dumbest, nonsensical things in order to avoid upsetting the man in charge. The culture is terrible and turnover is a real problem. Nothing is consistent...your boss and HR will tell you one thing, and you can see countless counter examples to what they said all across the org.
